Home and Well - NHS Direct Referral Service

 

The Home and Well Project is a collaborative partnership between West Hampshire CCG, SSEN, Southern Water and Portsmouth Water who are all active partners in the project. We support the most vulnerable; recognising in particular the importance of our service for people with mental health issues. 


The Home and Well NHS Direct Referral Service will be an additional service strand adding to the existing Home and Well core service and PSR strands. This Home and Well Remote Service will offer support with:


  • Help with signing up to the Priority Services Register (PSR) for Electric and Water supplies. This will mean that the patient receives a priority rapid response during any outage
  • An initial assessment of the patient's home circumstances regarding fuel poverty
  • An ‘initial check’ of the Patients social needs such as their financial position, housing needs and other support that surrounds them
  • Where necessary, connecting the patient to specialist partners within their local community to ensure they are provided with the "wrap around" support they need. We aim to make contact within 3 working days from the date of referral; this timescale may extend due to the current challenges we face due to the Coronavirus



This bespoke service is being supported by the NHS in particular to support patients with mental health issues. These may be from acute units or from any other NHS community provision. In particular the project will support:


  • Discharge from in-patient units across the county, by ensuring support is given to make sure utilities are connected and social needs addressed
  • Clients in the community, to ensure their employment, utilities, housing and finances issues are addressed
